IVP Level One Fingerprint Card Guide

Step 1: Submit Online Application:

  • Go to psp.azdps.gov and select on “Fingerprint Clearance Card”
  • Select “Apply for a Card/Request a Replacement”
  • Create an account by selecting “Don’t have an account? It’s easy to create one.”
  • Select “Apply for A New Clearance Card” & Select IVP
  • Fingerprint card type/reason for card: Select the one that applies to your position:
    • Bus Drivers:ARS 28-3228 School Bus Driver
    • Hourly Employees:ARS 15-512 Public and/or Charter School Non-certificated
      personnel
    • Teachers or other Certified Employees: ARS 15-534 State Board of Education
      (Teacher or Other Certification).
    • Include your social security number and the AZ Department of Education
      as a sponsor.
  • Once the application is submitted, applicants will be provided with an IVP Reference
    Number for use when getting fingerprinted. They will be asked if they want the
    fingerprints to be printed electronically. Select printed electronically.

Step 2: Fingerprint Appointment

  • Visit www.fieldprintarizona.com to schedule your fingerprint appointment. You will
    need the reference number starting in IVP that was given to you in your DPS message
    center. Select the Reason: Fingerprint Clearance Card.
  • After your fingerprint appointment is complete, DPS processes the IVP fingerprint card.
    • You can check the status of your card online psp.azdps.gov.
    • Once the card is issued you will receive the physical card in the mail. Please
      provide HR a copy of your card.
