Already a QCUSD Family?

Already a QCUSD family and need to enroll an additional student? Add another student through ParentVUE.

To enroll you need the following documents:

  • Original birth certificate
  • Proof of immunizations
  • Proof of residency (utility bill)
  • Records from previous school (if transferring)
  • Parent photo ID
  • For junior high and high school students, please fill out a course selection form for the school you are enrolling in and turn it in with the required documents. You can find forms on this page (to the right of the page on desktop, at the bottom on mobile.

During the online enrollment process, you will be able to submit the required documents electronically. Forms must be submitted before the start of the school year. 

If you are unable to use the online portal to enroll your child, you can download an enrollment packet below and return it to your child's school with the appropriate documents.

School Enrollment Packets

    Outside QCUSD Boundaries

    Do you live outside QCUSD boundaries or want to go to a school that is not your home school? Please fill out an open enrollment form and turn it into the school you are applying for. You will need to wait for approval before filling out the enrollment packet. Just fill out our open enrollment form!

    Kindergarten Enrollment

    QCUSD offers free all-day kindergarten at all of our elementary schools. A child is eligible for admission to kindergarten if the child is five years of age by August 31 of the current school year. If a child turns five between September 1 and December 31 of the current school year, he/she is eligible to apply for early kindergarten entrance testing.

    Preschool Enrollment

    We are pleased to offer students within QCUSD the opportunity to participate in preschool and prekindergarten. We have a variety of programs designed to suit your child's specific needs. Please visit our Preschool page for more information on programs and enrollment.

    Immunization Information

    Current state law mandates that all children entering school receive vaccinations against the following diseases:

    • Diphtheria, Tetanus, and Pertussis
    • Polio
    • Measles, Mumps, and Rubella (MMR)
    • Hepatitis B
    • Varicella
    • Hepatitis A

    Immunization Exception

    State law does allow exemptions from immunizations on an individual basis for reasons of conscience or medical or religious reasons. You must complete an ADHS exemption form to have your child exempted from immunizations. Please be aware that we will not allow students who are not current on their vaccinations to be in school if there is an outbreak of disease.
