Current state law mandates that all children entering school receive vaccinations against the following diseases:

• Diphtheria, Tetanus, and Pertussis
• Polio
• Measles, Mumps, and Rubella (MMR)
• Hepatitis B
• Varicella
• Hepatitis A

State law does allow exemptions from immunizations on an individual basis for reasons of conscience or medical or religious reasons. You must submit a written request to have your child exempted from immunizations. We’ve included the necessary form in the sidebar at the right of this page. Please be aware that if your child is not current on his or her immunizations, we will not allow your him or her to be in school if there is an outbreak of disease.
